To calculate the total sum for the Sudbury Vertigo Risk Score to estimate the probability of a serious central cause of vertigo, dizziness, or imbalance in adult patients presenting to emergency or urgent-care settings.

Use to stratify the risk of a serious central cause of vertigo in adult patients (≥18 years) presenting to an emergency department or urgent-care setting within 14 days of symptom onset. Apply only if the patient is: Alert (Glasgow Coma Scale = 15) Haemodynamically stable (SBP ≥ 90 mmHg) Without recent head or neck trauma, syncope, or active cancer The score is calculated by adding the points of the individual variables: Male: No 0 points, Yes 1 point Age > 65 years: No 0 points, Yes 1 point Diabetes: No 0 points, Yes 1 point Hypertension: No 0 points, Yes 3 points Motor or sensory deficit: No 0 points, Yes 5 points Cerebellar deficit (diplopia, dysarthria, dysphagia, dysmetria, or ataxia): No 0 points, Yes 6 points BPPV diagnosis (protective): No 0 points, Yes -5 points The total score estimates the probability of a central cause of vertigo and guides the need for further evaluation or neuroimaging.

Not to be used for recording information related to other vertigo or dizziness risk assessment tools or for patients who do not meet the clinical criteria for Sudbury Vertigo Risk Score application (e.g., altered mental status, unstable vitals, or recent trauma).
